PROCESSING YOUR ORDER
WHAT TO DO
DOCTOR INSTRUCTIONS:
Print out our online prescription form and fill out the necessary order information. Be sure to include the doctor's signature and license number. Include the signed prescription form with the case. Be sure to keep a copy for yourself.
ACCURATE MODELS:
We ask that you examine each model before sending it as the appliance we make is only as good as the model sent to us. Please check them for distortions and missing structures. Air bubbles or holes on missing tooth surfaces are unacceptable as they can negatively affect the fit of the appliance.
**Please no untrimmed models.
Accurate construction of bites is very critical in the proper fabrication of an appliance.
When it is necessary for brackets to be carved off the models before making the appliance, please remove the arch wires before taking the impression.
When carving is necessary, please send plaster models rather than stone.
***An accurate model trimmed on a solid base is essential for the construction of the best appliance possible.
PROCESSING YOUR ORDER FOR HABIT APPLIANCES:
Be sure to include both upper and lower models as well as a wax bit to insure a proper fit without interference of opposing arch.
PACKAGING MODELS:
Please be sure to wrap each model carefully, add extra foam or bubble wrap bvetween models and send them in a strong box to insure they arrive in good condition.
SENDING YOUR CASE:
For your convenience, we have included a printable postage paid label. Simply print it out, attach it to the box and give it to the mailman.
A NOTE ABOUT COST:
An accurate model trimmed on a solid base will keep shipping costs down.
TURN-AROUND TIME:
Once received, your case will require 5 working days in-lab.
